The goal is to have this show only one page. Any suggestions on how to make this happen?
If I remove the last bullet, it shows as one page.

This issue may occur because you have a few extra pixels on your last page that you convert. They may not fit into one page and that’s why those unwanted blank pages are created.
Please try to indicate to the Custom Format width/height the dimensions are bigger than your actual elements w/h. For example, your element has 595x842px, try to set 595x845px in the Custom Format field.
Also, as I can see from the screenshots you are using “custom format” but the format field is “a4”. If you want to be able to control your result doc’s width/height, instead of Format = a4 you can use Custom format as a format (Screenshot by Lightshot) and indicate your group/s width height in the Custom Format field to get the PDF file matching perfectly to your group parameters (w/h).
In case the issue still persists, please provide us with access to your app with edit rights (we will create a separate test page to perform plugin tests on it, and copy the page where you have faced the abovementioned issue).
For this please add our support@zeroqode.com email as a collaborator of your app (disregard the warning, our email can be added to the collaborator list even if your app is on the Free plan). And please let me know the page name where the issue is found.
