Maybe I’m just getting tired and this is obvious, but I am stuck trying to filter products by Category.
In the data type Item the “item_category” is a Category which is its own data type.

Yes, on our side it is configured in exactly this way.
Category type stores information about product categories. Item data type stores all the information about an item on the platform.
In case you want to add a new variable for sorting, please access your Data Section and select Option Sets.
Here you can create a new Option Set and sort your products by Category. A new type of sorting will appear in the dropdown on the Category page:
